This license is used for buried utility lines crossing the
county road, or for buried lines paralleling the county road for distances
of 1000 feet or more. For paved roads the buried line must
be bored. For gravel or dirt roads the buried line may
be trenched. Minimum depths are 3 1/2 feet below the ditch flow lines
or 5 1/2 feet below the crown point of the road.

This license is used for utility lines crossing the county
road right-of-way overhead where no boring or trenching is required and
for utility lines paralleling the county road for less than 1000 feet.
The minimum depth is 3 1/2 feet.

Permit - $50.00

This permit is used for a new road accessing onto a county
road. This is also used for approving any work or activity performed
within the county road right-of-way that is not covered by other permits
or licenses. Examples include: minor work on a utility line
in the county road right-of-way, placement of a sign or signal in the county
road right-of-way, placement of a mailbox, and overweight vehicles.

Address Application - $25.00

This application is required in order to assign a new county
address for a new building or location in the county. An address
is required for every residence, permanent building, or temporary building
at a work site. An address must be obtained before utility companies
install their utility lines.

Note: All four forms require a site
detail drawing to be submitted along with the form. In addition,
a section tie with bearing and distance from the nearest section corner
to the actual utility line crossing of the road is required for County
Licenses "A" and "B".
